Heart of Hope Homemaker and Companion Services is required by the Florida Agency for Health Care Administration (AHCA) to conduct a Level 2 background screening through the AHCA Clearinghouse for all employees. A complete SSN and Date of Birth is required to process this screening. Your SSN will be collected through this secure application form and used solely for the purpose of background screening. By submitting this application, you authorize Heart of Hope Homemaker and Companion Services and its authorized representative to conduct a Level 2 background screening in compliance with AHCA regulations.
